Isotretinoin is an oral retinoid derived from vitamin A and is primarily used for the treatment of severe acne and acne that has not responded adequately to conventional therapies. It reduces sebaceous gland activity and sebum production while influencing abnormal follicular keratinization, helping to reduce the formation of acne lesions. 

Effects

Isotretinoin significantly reduces the activity and size of sebaceous glands, resulting in a substantial reduction in sebum production. This action helps decrease the conditions that contribute to clogged pores and the development of acne lesions.
Isotretinoin also influences follicular keratinization and may reduce the formation of comedones. By affecting several mechanisms involved in acne development, it can help reduce both inflammatory and non-inflammatory lesions and may contribute to sustained improvement following completion of treatment.

Side Effects

Potential adverse effects associated with isotretinoin may include dry lips, dry skin, dry eyes, nasal dryness, skin sensitivity, and increased sensitivity to sunlight. Headache, muscle or joint discomfort, changes in blood lipid levels, and elevations in liver enzymes may also occur.
More serious risks are possible, particularly with inappropriate use or in individuals with specific medical conditions. Isotretinoin is highly teratogenic and can cause severe birth defects if used during pregnancy.

Dosage Range and Duration of Use

Isotretinoin 20 mg: Each capsule contains 20 mg of isotretinoin. Standard clinical protocols generally start at a daily dose of 0.5 mg per kilogram of body weight, increasing to 1.0 mg/kg daily as tolerated, until a target cumulative dose of 120 mg to 150 mg/kg is reached over a treatment duration of 15 to 20 weeks. Treatment is administered in one or two divided doses daily with high-fat meals to optimize absorption. Dose adjustments may be required based on clinical response and side effects, and a second course may be considered after a 8-week break if severe acne persists. 

Combination with Other Drugs

Isotretinoin may interact with medicines and supplements that affect vitamin A metabolism or have overlapping adverse effects. Concomitant use with tetracycline antibiotics may increase the risk of intracranial hypertension, while combining isotretinoin with vitamin A supplements may increase the risk of vitamin A toxicity.
